Epidural Steroid Injection vs Paravertebral O2O3 Infiltration for Symptomatic Herniated Disc Refractory to Conventional Treatment

چکیده
Backache, with or without sciatica nerve or femoral involvement, affects 90% of the population at least once in their lives and is one of the major causes of working days lost in the western world . Until 15 years ago surgery was the treatment of choice. Nowadays the trend is to adopt conservative treatments given the many limitations of surgery in terms of short and long-term pain resolution . T The different methods used to treat low back pain and sciatica caused by disc and nondiscal disease (osteoporosis, facet joint syndrome, canal stenosis, etc.) include epidural steroid injections and paravertebral infiltration of O2O3 gas mixtures . The aim of this study was to compare the therapeutic efficacy of these two procedures in a randomized prospective study of patients with acute or chronic low back pain with sciatica/cruralgia caused by herniated disc. Effectiveness of epidural steroid injection for the management of symptomatic herniated lumbar disc.
Low-back pain is a common clinical presentation of herniated lumbar disc. This is the most common presenting complain of the young adults. The incidence of low back pain is high in our part of the world. The reason may be hilly terrain, difficult working and living environment. The initial treatment of low back pain is conservative. Epidural steroid injection (ESI) is being slowly established a...

STUDY DESIGN Multicenter randomized, double-blind, simulated therapy-controlled trial in a cohort of patients with acute low back pain (LBP) due to lumbar disc herniation (LDH). OBJECTIVE To assess the benefit of intramuscular-paravertebral injections of an oxygen-ozone (O2O3) mixture. SUMMARY OF BACKGROUND DATA Recent findings have shown that O2O3 therapy can be used to treat LDH that fail...
متن کاملLumbar disc herniation regression after successful epidural steroid injection.
In some lumbar disc herniation patients, noninvasive measures fail, necessitating more aggressive treatment, such as epidural steroid injections or surgery. This study sought to determine whether improvement in patients who receive epidural steroid injections is related to regression of herniated nucleus pulposus or whether such patients' symptoms decrease because of the steroid effect in the p...
متن کاملTreatment of lumbar disc herniation: epidural steroid injection compared with discectomy. A prospective, randomized study.
BACKGROUND Epidural steroid injection is a low-risk alternative to surgical intervention in the treatment of lumbar disc herniation. The objective of this study was to determine the efficacy of epidural steroid injection in the treatment of patients with a large, symptomatic lumbar herniated nucleus pulposus who are surgical candidates. METHODS One hundred and sixty-nine patients with a large...
متن کاملThe impact of epidural steroid injections on the outcomes of patients treated for lumbar disc herniation: a subgroup analysis of the SPORT trial.
BACKGROUND The Spine Patient Outcomes Research Trial (SPORT) is a prospective, multicenter study of operative versus nonoperative treatment of lumbar intervertebral disc herniation. It has been suggested that epidural steroid injections may help improve patient outcomes and lower the rate of crossover to surgical treatment. METHODS One hundred and fifty-four patients included in the intervert...
